How to Make an Appointment
VerityMD Neurology Clinic is a referral-based practice. To ensure we provide the best care, we require a formal referral from your healthcare provider before scheduling an appointment.
1 Obtain a Referral
Before scheduling an appointment, we need a formal referral faxed to our office at (916) 244-3875 directly from your referring provider. This is not an insurance referral/authorization but a referral for services.
The referral must include:
The reason for referral.
Complete patient demographic and insurance information.
All relevant medical records, including test results related to the reason for referral.
2 Insurance Authorization (If Required)
If your insurance plan requires prior authorization (e.g., HMO policies), we cannot schedule your appointment until we receive the appropriate and complete insurance authorization for services.
It is the patient’s responsibility to contact their PCP and ensure the authorization is faxed to our office.
If an appointment is scheduled without the required authorization on file, you will be considered a self-pay patient and will be responsible for all charges.
3 Scheduling Your Appointment
Once we receive the complete referral and insurance authorization (if required), our team will review the information and contact you to schedule the appropriate appointment type.

Self-Pay Policy: If your insurance authorization is not received before your visit, you will be responsible for payment as a self-pay patient.
