How to Become a New Patient at VerityMD Neurology

VerityMD Neurology is a referral-based neurology clinic. New patient referrals must be faxed directly from the patient’s primary care physician, specialist, or referring provider’s office.

This process helps our team review the reason for referral, verify insurance requirements, triage the concern appropriately, and schedule the patient with the right provider or testing pathway.

For Patients

If you would like to be seen at VerityMD Neurology, please contact your PCP, specialist, or referring provider and ask them to fax a neurology referral to our office. Please do not send medical questions, records, forms, or appointment requests through the website.

What the Referring Office Should Send

Referrals should include patient demographics, insurance information, reason for referral, relevant notes, medication list, recent labs, imaging reports, and prior neurology records if available.
