VerityMD Neurology partners with primary care physicians, specialists, hospitals, and referring provider offices to provide neurological evaluation, diagnostic testing, and care planning for patients with a wide range of neurological concerns.

Please fax referrals directly to VerityMD Neurology. The website is not used for patient-uploaded records, online intake forms, clinical messages, or appointment requests.

Diagnostic Services Available

VerityMD offers routine EEG, ambulatory video EEG monitoring, EMG/NCV testing, autonomic nervous system testing, small fiber neuropathy testing using TM Flow, cognitive screening, Creyos cognitive testing, and neurological medication review when clinically appropriate.

Please Include With Referrals

Include demographics, insurance information, reason for referral, relevant office notes, medication list, recent labs, imaging reports, prior neurology records, hospital records if relevant, and prior EEG, EMG/NCV, autonomic testing, cognitive testing, or sleep study results if available.

  • Improves accuracy of triage

  • Helps schedule appropriate provider or testing pathway

Condition-Specific Referral Notes

For Botox or migraine referrals, include prior medication trials when available. For dementia referrals, include family concerns and prior cognitive testing when available. For stroke/TIA referrals, include hospital records, imaging reports, vascular imaging, echocardiogram, heart monitor results, and discharge medications when available.
